This Privacy Policy describes the information Dr. Fujimoto Optometry Professional Corporation operating as Dr. Fujimoto (“Dr. Fujimoto“, “us” or “we“) collect from you and how we use that information.

1. What personal data we collect and why we collect it

a. With your consent

Any information that you voluntarily input on our website implies that you have given us consent to use and store your personal information.  For example, when you use our “Contact” form on our website, you are voluntarily giving us consent to store your personal information you added into the form and that you have read and agree to our Terms of Use and Privacy Policies (this policy). Once email is transmitted from our secure encrypted website servers onto the internet, the information may not private.  If you are concerned about confidentiality, it is best to talk to us in person or phone us directly instead of using our “Contact” form.

b. Withdraw your consent

You may withdraw your consent at anytime by contacting us to delete your personal information on file subject to retention laws in force.  We will immediately stop using your information as soon as possible after we confirm with you what the consequences are of deleting your information from our systems. eg. email addresses or contact numbers equates to no more appointment reminders or electronic billing receipts.

2. Comments

We may in the future have a blogging section where visitors can leave comments on the site.  We may collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help with spam detection.

If we have a future blogging section, an anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/.  After approval of your comment, your profile picture is visible to the public in the context of your comment.

3. Media

If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included.

When you use our “Contact” form on our website, you are voluntarily giving us consent to store your personal information you added into the form and that you have read and agree to our Terms of Use and Privacy Policies (this policy). When visiting our website your browser may collect statistical information using cookies.  A web cookie is a small file saved onto your computer by your browser when visiting a website.

When browsing Dr. Fujimoto’s website, cookies are used to collect anonymous statistical information such as browser type, screen size, traffic patterns and pages visited. This information helps provide you with better service and experience when using our website.  We do not store personal information in cookies, nor do we collect personal information from you without your knowledge.

Embedded content:   Articles on this site may include embedded content (e.g. videos, images, articles, etc.). Embedded content from other websites behaves in the exact same way as if the visitor has visited the other website.

8. Who we share your data with

The information you provide will not be used as marketing tool but rather to respond to your request for information, appointment reminders, or for receipt/invoice purposes. We will not sell or distribute your information to any third parties.  If we sell our business, however, we reserve the right to transfer your information as well as our assets and data on those assets to the new owner of the business as is.

There are limited circumstances where information can be used without consent when:

  • it is clearly in the person’s interests that person’s consent cannot be obtained in a timely way, or that person would not reasonably be expected to withhold consent.
  • there is a legal authority for the use as is authorized by a statute of Ontario or Canada, a regulation of Ontario or Canada, a bylaw of a local government body, or a legislative instrument of a professional regulatory organization.
  • it is for an inspection or audit if the audit is either of or by the organization and is authorized by a statute or regulation of Ontario or Canada or if it’s by another organization and it’s not practical to use non-identifying information for the purpose of the audit.

  • it is necessary to collect or pay a debt owed to Dr. Fujimoto.

  • where legally bound by law, to provide your information to law enforcement agencies to prevent fraud, abuse, or to protect the interests of Dr. Fujimoto and its owner, clients, contractors, and employees.

All our web services remain in Canada and on Canadian IT Infrastructure, and are not subject to foreign policy or laws forcing us to disclose your personal information that you voluntarily provided to us, to foreign entities.

9. What rights you have over your data

You can also request that we erase any personal data we hold about you.  This does not include any data we are obliged to keep for administrative, legal, or security purposes.

Visitor comments may be checked through an automated spam detection service.

a. How we protect your data

Protecting the privacy of your personal information is important to us.

Dr. Fujimoto takes reasonable security measures against unauthorized access, collection, use, disclosure, copying, modification, disposal or destruction of information.  We limit the amount of personal information we collect in the first place making security easier.

We have developed policies and practices including those that protect personal information.  They include but not limited to the following physical security, technological security, such as password protection, administrative security, such as confidentiality agreements and terms of use for information technology, and how we respond to inquiries and complaints.

b. Regulatory disclosure

We conform to the principles of the Personal Information Protection and Electronic Document Act (PIPEDA) of Canada.  Ontario has its own privacy act called Freedom of Information and Protection of Privacy Act (FIPPA) and the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA) for which we follow.   We collect, disclose, and use your personal information for the purposes that any reasonable person would consider appropriate.
